DMEK surgery pioneered by Dr. Melles provides excellent visual outcomes. However, it has many challenges including orientation, centration, unrolling and applanation of the descemet-endothelial complex. A novel DMEK punch will be presented which cuts a graft with a tail. The tail is used to pull the graft into the eye, orient, centre and manipulate the graft. The tail is also helpful in re-injecting gas if the graft is detached postoperatively.
