A staged strategy for misdirection syndrome in cataract surgery is outlined. Starting with a Honan balloon or osmotic agents, progressing to aqueous drainage by sclerotomy or anterior vitrectomy, and, in severe cases, posterior vitrectomy with hyaloidectomy, double capsulectomy, and iridectomy.
