We present an innovative approach to complex cataract surgery with iris coloboma repair in a patient with multiple ocular anomalies, including microcornea and lens coloboma. Key steps include strategic viscoelastic use, precise capsulorhexis, capsular hooks, CTR, and toric IOL implantation. Iris repair involved iris sutures and intraocular diathermy.
