A 76-year-old female presents for cataract surgery. VA was 20/200 OD and HM OS. H/O RK 34 years ago, and pathological myopia. LOCS was NO4NC5. We demonstrate a scleral approach for phacoemulsification + IOL implantation. A posterior capsular metaplasia is revealed during surgery, so an PCCC is performed to avoid YAG-Laser capsulotomy in the future. VA at the moment 20/80.
