A 29-year-old female had ICL implantation following which some laser was done and now had total cataract with ICL in situ. USG revealed lens matter in vitreous chamber. UBM revealed a large posterior capsular defect with lens matter projecting posteriorly. Pars plana vitrectomy with lens matter removal with ICL explantation and a multipiece IOL implantation in sulcus was done with acuity of 20/25.
