Intraoperative Floppy Iris syndrome causes poor pupillary dilation, and iris prolapse leading to intraoperative complications during cataract surgery. The surgical pearls are long intra-corneal tunnel and short side ports, use of dispervise OVDs and be cautious not to overfill the chamber, minimal hydro dissection and iris touch also use low pressure and low flow rate for an uneventful surgery .
