Full-thickness keratoplasties are fraught with wound healing and optical problems. Laser-assisted wound creation is used to mitigate these challenges, but has it gone far enough? We think not! Long-term follow up of an “extreme mushroom” keratoplasty case, the underlying mathematics, and changes in laser technology and eyebank capabilities support a paradigm shift in penetrating keratoplasty.
