A 77-year-old female presented with a corneal ulcer that led to a massive central corneal melt. It was decided to perform urgent penetrating keratoplasty, which involved extensive separation of adhesions due to unclear anatomy. The surgery also revealed vitreous prolapse and aphakia. Six months after the surgery, the graft is clear with no recurrence of infection or rejection.
