Inadvertent digital microscope mirroring led to a near-miss scenario, averting an inverted LAL IOL. The IOL was indeed properly placed but confusion ensued among OR team members. Even an experienced surgeon may second guess themselves and induce an inversion when one was not present. We discuss the potential for digital OR imaging systems to mislead surgeons/team members and potential sequelae.
