Operation Sight is excited to present a one-link process for you to track your impact as you provide charitable cataract surgeries. Bookmark the form today to ensure you're tracking your practice's impact.
The "Track Your Impact" digital form below should be completed post-surgery for each Operation Sight patient. This ensures your practice's impact is being appropriately recognized and recorded! In only a few minutes, you can complete the form to notify Operation Sight of your practice's volunteerism. The program's recognition opportunities will include regular features on the ASCRS Foundation website and social media.
Please note the form should be completed per patient not surgery. If a patient is scheduled for both eyes on separate surgery dates, please submit the form after the last surgery. We kindly ask that forms are received no later than 4 weeks after the surgery date has occurred. You can submit the form for surgeries that have occurred in 2021 thus far.
