A patient presented with a dislocated lens in an acellular capsular bag, completely devoid of capsular scar tissue. The posterior capsule was noted to be wrinkled and diaphanous in appearance, with an almost gossamer-like quality. A three-piece intraocular lens in the sulcus with optic capture was achieved. This represents a classic case of dead-bag syndrome, a rare cause of late lens dislocation.
