Acute intraoperative aqueous misdirection is a rare, vision threatening occurrence in cataract surgery. In our video we discuss the protocol for reaching this diagnosis, the settings and technique for the procedure along with outcomes in a small series of cases. IZHV, when performed with correctly can be an excellent solution for acute aqueous misdirection for anterior segment surgeons.
