Cataracts with fibrous anterior capsules challenge the cataract surgeon to make a complete continuous circular capsulorhexis. Failure to do so creates difficulties during surgery and to IOL positioning. This film considers the surgical and structural aspects of fibrous anterior capsules, looks at associated novel biomarkers and considers how this information may help in reducing capsular fibrosis
