42 y/o with 10 years of pain, blurred vision and intraocular hemorrhages in the RE presented with VA of 6/15 CC. History included RE cataract surgery in childhood due to congenital cataract. Examination revealed an AC 3P IOL with haptics in iris and an irregular pupil. Surgery included IOL removal, synechiolysis, sulcus IOL implantation, and pupilloplasty. 6 weeks post-op VA improved to 6/9 CC.
