A 37-year-old woman with congenital aniridia and bilateral cataracts underwent right-eye cataract surgery using indocyanine green (ICG), along with a light pipe and coaxial light. Post-op, her vision improved to 20/350. The left eye remains at 20/800, with further treatment pending. This case highlights the innovative use of ICG in complex congenital aniridia cases.
