Phacoemulsification can be difficult and stressful in some situations, especially for beginning surgeons. We designed a new method called “cave and divide”. The method is primarily useful in a small pupil and/or billowing iris. It provides a good visual control over the instruments, iris stability, and an easy way to continue after the first unsuccessful attempt to split the nucleus.
