Manual small incision cataract surgery was performed in a 55-year-old male with uncontrolled diabetes and total cataract. Pre-operative visual acuity was light perception and axial length was 34.58 with poor pupillary dilation. An anterior chamber maintainer was used to maintain proper fluidics and an Alcon -3.0 D MA60MA lens was inserted with an aim of -0.11 D residual refractive error.
