A bilateral case of total pupillary occlusion and seclusion secondary to overly-aggressive iris enclavation from iris-claw IOL and its surgical management will be discussed. A patient presented with bilateral total pupillary occlusion and seclusion after PMMA iris claw IOL implantation. The surgical management for iris-claw IOL explantation, iris reconstruction, and clear lens exchange/cataract extraction will be discussed.
