Insufficiency or absence of the capsular support represents a big challenge for cataract surgery. I report the case of a young patient with traumatic cataract at the left eye. The surgical approach included phacoemulsification of the cataract and posterior chamber implantation of a 3-piece artificial lens with sutureless scleral fixation - Yamane technique, a suitable method to avoid surgery complications, which was performed under subconjunctival anesthesia.
