This video presents a case of severe capsular contraction after explantation of a Crystalens optic that developed Z-syndrome and subsequent centralization of the residual haptics. After removal of the haptics, intraoperative relaxing incisions were made in the anterior capsule to allow optic capture of a three-piece IOL. These incisions allowed for a stable centered IOL placement, retention of the capsular bag, and relief of zonluar tension.
