This presentation describes a new technique of managing posterior capsular tears during irrigation aspiration stage of phaco surgery. A posterior capsular tear can be managed by creating a posterior circular capsulotomy with the coaxial irrigation aspiration cannula by changing the parameters and not withdrawing the IA probe. This maintains the anterior chamber and prevents break in the anterior hyloid face and enables successful in the bag implantation of IOL. This technique is suitable for central tears without vitreous prolapse.
