Video reviews considerations and steps taken in managing a shallowing anterior chamber and hard eye just before cortical cleanup due to possible fluid misdirection syndrome. Proceeding with cortical clean-up without a vitrectomy seemed fraught with complications but a vitrectomy could worsen a possible suprachoroidal effusion or expulsive hemorrhage. The decision to strategically abort and revisit the case in a few days led to a good outcome in the end.
