Retained viscoelastic (OVD) under monoblock IOL was observed 4 years after Phacoemulsification. The Spherical equivalent (SE) was +6 D. Capsular bag was successfully opened with OVD and Tan DALK dissector. Old OVD had oily appearance. “Pac-man” technique was used for IOL removal through 3.2 mm limbal incision. The new 3-pieces IOL was placed in the capsular bag. Postoperative SE was +1 D.
