In the transition from Descemet stripping endothelial keratoplasty (DSEK) to DMEK, improvements in postoperative visual acuity have come with a more challenging surgical technique and less predictable intraoperative graft behavior. The surgical approach presented here addresses the fragility and unpredictability of the DMEK graft by making it as easy to manipulate as a DSEK graft.
