Cataract in a phakic IOL is a situation that calls for delicate surgery and perfect refractive correction. This video shows the procedure of cataract surgery in the presence of a posterior chamber phakic IOL, without explantation of the phakic IOL, and introduction of a toric multifocal IOL beneath the phakic IOL, the tips to be followed in such an unusual situation and the outcome. To our knowledge this is the first documented procedure of such a case.
