We present a case of a 58 year old man with bilateral hexagonal nuclear cataracts. This unusual but beautiful type of cataract brings up a discussion of embryology and anatomy of the crystalline lens and pathophysiology of cataract formation along lens suture lines. This film shows photos of the patient's interesting hexagonal nuclear cataracts as well as surgical videos of the cataract surgery in both eyes.
